Patients who have consulted Practitioner Lee know that during visits, I often ask you to "produce a few coughs for me to hear." Why listen to the cough sound? In Traditional Chinese Medicine (TCM) syndrome differentiation and treatment, the pitch, resonance, frequency, and origin of a cough offer vital clues regarding the depth of the illness, as well as whether it stems from Cold, Heat, Deficiency, or Excess!

I. 4 Common Types of Coughs and Their Sound Characteristics


1. Early-Stage Wind-Cold Cough

  • Cough Sound: Clear and crisp, originating shallowly from the throat.

  • Accompanying Symptoms: Itchy throat, sneezing, clear runny nose, and aversion to cold.

  • TCM Analysis: Caused primarily by exterior Wind-Cold pathogens invading the muscle layer and skin, impeding the proper dispersion of Lung Qi.


2. Early-Stage Wind-Heat Cough

  • Cough Sound: Clear and crisp, but with a noticeable hoarseness, originating shallowly from the throat.

  • Accompanying Symptoms: Sore and swollen throat, dry mouth, yellow nasal discharge, or fever.

  • TCM Analysis: Wind-Heat pathogens attack the Lungs and consume body fluids, resulting in a hoarse cough accompanied by throat redness and pain.


3. Lung-Heat Cough (Bronchitis Stage)

  • Cough Sound: Heavy, deep, and hollow-sounding, distinctly echoing from deep within the chest cavity.

  • Accompanying Symptoms: Profuse, thick yellow phlegm (or sticky phlegm that is difficult to expectorate), chest tightness, and shortness of breath.

  • TCM Analysis: Pathogens penetrate interiorly and transform into Heat, sinking deep into the Lung zang-organ. The Lungs lose their ability to purify and descend Qi, condensing fluids into phlegm. This represents a deeper Heat-pattern cough requiring timely clearing of Lung-Heat and resolving phlegm.


4. Allergic Cough (Post-COVID / Persistent Post-Flu Cough)

  • Cough Sound: Crisp, brief, and sudden coughing fits, with each episode lasting 1–2 minutes or longer.

  • Accompanying Symptoms: Phlegm and Cold/Heat symptoms have largely resolved, but the throat remains extremely ticklish. Exposure to cold wind, second-hand smoke, or strong odors triggers uncontrollable coughing.

  • TCM Analysis: This stage usually occurs at the tail end of an illness, where the body's vital Qi has not yet fully recovered and lingering "Wind pathogens" remain. Although pathogenic Heat has cleared, the airway hyper-reactivity leads to paroxysmal spasmodic coughing.

Q1: Can a chronic or lingering cough be resolved solely through dietary remedies or over-the-counter cough syrups?

A1: Most commercial cough syrups contain cough suppressants or antihistamines. Without syndrome differentiation based on individual constitution, they often treat symptoms rather than the root cause. For instance, taking cooling and moistening herbs for a Wind-Cold cough can trap the cold pathogen inside. Conversely, an allergic cough requires herbal formulations that "dispel wind, diffuse the lungs, nourish Yin, and stop spasms," combined with acupuncture at acupoints such as Yingxiang (LI20) and Lieque (LU7) for complete recovery.


Q2: What dietary restrictions should I follow while coughing?

A2: Regardless of the cough type, strictly avoid raw or cold foods, deep-fried or greasy dishes, sweets, and dairy products. Sweets and cold foods generate dampness and phlegm, burdening the Spleen and Stomach. Deep-fried items generate internal heat and damage Yin, aggravating throat inflammation and coughing fits.
